What is the Process to Begin Services?
Step #1: Initial Interview
Your first step will most likely be a phone call to our office where you will speak to a friendly SYNERGY HomeCare representative. We will gather pertinent information such as brief medical history and type of care needed. If you are new to home care we can help answer your questions and guide you in the right direction. We can also provide valuable resources if you or your loved one is not yet ready.
Step #2: In Home Consultation/Assessment
If you decide to move forward a SYNERGY HomeCare representative or Registered Nurse will conduct a no obligation free consultation/assessment to help determine the most suitable type of care. We will create a detailed care plan and learn more about you or your family member in detail so we can provide the most suitable caregiver based on personality and skills. We will also determine and finalize the days/ hours that services are needed and a start date. You can even meet the caregiver before the start of services.
Step #3: Services Begin
You can rest assured a skilled and friendly caregiver will be assisting you or your family member to live an independent and better quality of life. We will also continually keep in touch through telephone and/or home visits by a Registered Nurse to ensure quality service.
