I am so honored to serve the people in my home community. I have a Master’s degree in Adapted Physical Activity (SJSU), and have served in several organizations helping to make a positive impact on seniors, adults, and children of all ability levels and dimensions of diversity.
Personally, home care has touched my life in a significant way. For a number of years I have relied on in-home caregivers to support my own parents, Bob and Darleen Nelson (both retired teachers from the Campbell/San Jose area), who have faced many of the mental and physical challenges that accompany aging. Although their caregivers were very nice people and met their basic needs, I always felt there was more that could be done to help seniors thrive in their own home. Following this experience, and many years helping people with various disabilities, I was compelled to start a home care business that serves everyone.

Hello! I'm Lori Chaplin. I am so excited to be a part of this Synergy team because I am a passionate and caring person who has seen first-hand what a family can go through when a loved one needs care. My parent’s health changed, and we had to rely on others for their everyday care. I realized that the right caregiver would put their heart into making my parents happy, safe, and comfortable in their own homes. We can all make a big difference in other's lives, and it starts with understanding each human and family we interact with.
As the operations manager, I will apply my 25 years working with high tech companies in employee/customer relations, project management and communications. My focus is to ensure our caregiver, client, and family communications are responsive, transparent, and proactive.

My interest in the caregiving field developed this past summer when I was blessed with the chance to help care for a friend’s grandma. I really valued and loved my time with her and the things I learned through that process were life changing for me. I am excited to support our caregivers and help empower them to give our clients the best care they can!
Professionally, I have worked in many different areas throughout my career including human resources, training and development, and wellness for a health and fitness company. I currently hold certifications in Human Resources, and as a Health Coach with the American Council on Exercise.
Toni Toledo, MPH RDN
Nutrition/Wellness Education Coordinator
I am a registered dietitian, wellness educator and Campbell native. I have worked in various health promotion roles for over 25 years and currently work at the Palo Alto Medical Foundation in the behavioral weight management program. Personally, I’ve experienced the positive impact caregivers had with my grandparents. I saw first-hand how that blessing impacted not only them, but their entire family benefited from that piece-of-mind knowing they were in good hands. I will be supporting our Synergy caregivers and clients by providing training and resources in wellness, healthy meal choices and preparation.
My latest passion is my podcast –PATH Positive Approaches To Health. PATH explores inspiring, interesting and practical examples of what is going on around our nation that helps people achieve their best health. My hobbies include traveling, goofing off with her friends and family, or jogging along the Los Gatos creek trail.
I specialize in writing and multimedia production. I received my Bachelor’s degree from the University of Southern California, where I honed my skills in written and visual storytelling. I have worked on the frontlines of customer experience for many years, and have developed a deep sense of responsibility to take care of my clients and colleagues. I believe that home care is a life-changing resource that deserves greater awareness.
