Step 1: Getting to Know One Another

Selecting the right home care agency can be a difficult process. Our experienced and friendly staff will help you sort through the many options, compare costs, and get you help quickly. We understand that the most important consideration in finding care for a loved one is feeling comfortable placing your trust in the agency providing the caregiver. Another important factor is cost. We do everything we can to keep costs in line with your expectations. We can assist with Insurance policies, utilize monitoring devices, and customize a care plan that provides the specific coverage you need in order to reduce care costs.

In addition, we always proceed at your pace. We offer an entry level option called Courtesy Care, which provides remote check-in and periodic visits to ensure you or a loved one is out of bed, dressed, medications taken and day begun. If immediate service is required, we pledge to provide a caregiver within 4 hours.

You Have Options.

At SYNERGY HomeCare, we understand that often the simplest form of help can mean a great deal to those in need. For this reason, we offer a variety of levels of assistance, from 4 hours a week, to full-time care. Home care services may include any or all of the following:

  • Courtesy Care
  • Basic housekeeping
  • Assistance with bathing, dressing and personal hygiene
  • Meal preparation
  • Transportation to/from doctor's appointments or errands
  • Medication reminders
  • Specialty care (dementia, Parkinson's, Quadriplegic, etc.)
  • Companionship, reducing depression and calming anxiety

What is a home assessment? During a no-obligation SYNERGY HomeCare assessment, we'll confirm your needs, explain home care options, compare costs of long-term care and help you to decide what's best for you or your loved one, whether it be providing care at home, in a nursing home, or at an assisted or independent living facility. During the conversation we:

  • Review your current situation
  • Categorize your needs and wants
  • Evaluate your environment
  • Develop a customized care plan

Step 3: Individualized Care

Once the consultation is complete, our home care services may commence immediately.

At SYNERGY HomeCare of Northern Chicago, we utilize stringent employment requirements and supplement the knowledge of our caregiver staff with top in the industry advanced caregiver training. Additionally, we perform extensive caregiver evaluations to ensure compatibility with you or your loved one.  Our mission is to provide in-home care of the highest quality by matching you with a caregiver who is much more than a non-medical helping hand. We strive to find caregivers who understand that they are an integral part of and compassionate presence in your family’s life.

Step 4: On-Going Support

We understand that circumstances change, so here at SYNERGY HomeCare, we are committed to serve as your care partner for you or your loved one. Whether four hours a week or 24 hours a day, our caregivers are trained to be attentive to changes in habits or routines and to notify our Case Manager immediately. The communication process is then activated, and adjustments are made with full consensus.

Continuing communication is a critical part of administering effective home care. After we complete our initial home assessment, we accompany our caregiver on the first day to make a warm introduction, we follow up at the conclusion of the first day to confirm that care was as expected and continuing days as preferred. Regular monthly calls and quarterly visits further safeguard that our care is top quality. We also offer Family Connect a web based tool designed to collect daily care notes and provide approved family or medical personnel same day care updates.
