Free Assessment & On-Going Care Management 

  • free assessment at client’s home or on-site at facility (rehab, for example)
  • coordination of care plan with client, family & health care professionals
  • on-going care management through communication with caregivers, client, family, & professionals
  • advice on reimbursement sources & other beneficial resources* 

Meals 

  • assist with meal planning
  • grocery shopping
  • meal preparation
  • assist with feeding 

Light Housekeeping 

  • maintain clean/healthy environment throughout home
  • wash clothes/laundry
  • change/make beds
  • dust/vacuum/sweep/mop
  • empty trash 

Companionship & Supervision 

  • socialize/converse with client
  • go for walks
  • assist with planning activities
  • encourage social activities & hobbies 

Personal Care Assistance 

  • assist with morning &evening routines
  • personal hygiene assistance including:

      - bathing/showering/dressing
      - toileting/incontinence care

  • medication reminder
  • assist with transfers/hoyer lift use
  • assist with prescribed therapy 

2 Hours to 24-Hour & Live-In Care 

  • schedule & service customized to meet client’s needs
  • caregivers experienced with dementia & hospice care
  • respite care 

Errands & Transportation 

  • accompany to medical appointments, hair appointments, etc.
  • pick up prescriptions

Courtesy Calls

Courtesy Calls can be provided in addition to other services or for clients who prefer no other services. Provides client with caring connection and family with peace of mind.

Regular check-in calls to client followed by transcript of call via email to family (includes at least one face-to-face visit)

SYNERGY HomeCare will partner with your chosen home hospice provider to provide: 

  • care & comfort for the client
  • respite for the family
  • meal planning & preparation
  • daily support & supervision
