How Do We Start Service with SYNERGY HomeCare?
You decide how you want to initiate the request for information about how SYNERGY HomeCare can provide home care services for you or a loved one. Either way, the first phone call will be an opportunity for you to explain your situation to us, ask us questions about SYNERGY HomeCare and our services, and arrange for the best time for our owner or Director of Nursing to come to your home to meet with you for a Free In-Home Care Assessment.
When we come to meet with you, we will want to meet with the care recipient, as well as the family member or friend who is in involved in making the health care decisions. During this meeting, we will discuss medical history, the reasons why home care is being requested, the services being requested, and any information relevant to the care recipient’s care. We will also perform a safety check of the home and make recommendations on how to make the home safer. And if requested by the client, we can make arrangements for the client to interview one or more potential caregivers. During the meeting, we will create a detailed Plan of Care which will be used by the SYNERGY HomeCare staff to select the caregiver that best meets the needs and personality of the care recipient.
On the first day of service, the owner, the Director of Nursing and the caregiver will all arrive together to meet with the client and family/friend. The Director of Nursing will do a Nurse Assessment, will create an updated Plan of Care and will ensure that the client, the caregiver and the family are all in agreement on the services that will be provided. The nurse will also review the medications that the client is taking and review them with the caregiver if we will be providing medication reminders.
Once service has begun, the SYNERGY HomeCare staff will stay in constant contact with the client to ensure they are happy with the caregiver(s) and the services being provided. And our Director of Nursing will be following up at least once a month to make sure the client is receiving excellent care.
