Getting Home and Staying Home
Have you ever spent an extended amount of time in a hospital these days? Well if you have whether you are relatively healthy or in poor condition, it is not the most pleasant experience. Don't get me wrong hospitals do a great job diagnosing your ailments or just fixing whatever ails you at the time. Hospitals are not equipped to be the Ritz Carlton or your comfortable home. They are meant to give the physician a clinical environment, equipped to provide the highest level care you need when sick. With that said if you do find the need to visit the local hospital in your area, you really want to have them diagnose and heal, and then get you on your way home. If you happen to be a senior and have the luxury of using Medicare, Hospitals have an incentive to get you well and get you home. The great thing about Medicare is that the majority of your care expense is picked up by Medicare. Although Medicare has its advantages, Hospitals will be under a microscope come the fall of 2012 regarding your condition after you return home from a hospital visit. Between the Medicare Plan and the Hospital incentives, the patient will be caught in the middle of a debate as to the care plan after you return home.
The perfect scenario would be for the hospital to completely heal you, provide you with information on how to care for yourself when you get home, and a family or friend to make sure this plan is followed. As mentioned before I can't think of anyone that would want to make a revisit to the hospital only because the plan structured by the hospital or your doctor was not followed. Since hospitals and Physicians have a direct incentive for you not to return to the hospital they will provide you with alternatives to family and friend Caregivers in your home should family or friends not be available. These professional caregivers can insure you follow the structured plan laid out by the medical professionals.
Synergy HomeCare of Montgomery and Bucks County is one of many Home Care providers in the area that can provide this service to you or a loved one. Unfortunately Home Care is not covered by Medicare and must be an expense directly paid by you the consumer. Long Term Care Insurance companies do cover this expense should you need the care at home, but you would have planned for this insurance well before you needed the care. So you the consumer and recently discharged patient needs help and have limited funds, no local family and friends that are just too busy to help you during the day. Synergy HomeCare has developed service plans that may fit into your limited budget and with that, help you remain at home, heal, feel better and avoid a revisit to the hospital.
Traditional care is a Caregiver visiting you maybe 3-4 times per week, providing assistance with your personal care. This can include meal prep, light housekeeping, walking assistance, errands, and med reminders. This Service can range anywhere from $18.00- $25.00/hr depending on the care and experience needed. Synergy HomeCare has this traditional option available to you along with other more economical options. Courtesy Call Service can provide a 15 minute, recorded call to your loved one on a set, scheduled basis. This call is scripted and will inquire as to the emotional and physical condition of your loved one. The call can also verify that tasks that were part of the Hospital discharge plan have been followed insuring the visit back to the hospital be avoided. This call since it was recorded can be sent to a loved one should they not be local. Courtesy Visit Service will provide a 1 hour visit to your loved one to physically check that tasks are being done as directed by the hospital. These services can be used in combination and also utilized with traditional Personal Care Service. Courtesy Calls can be provided at $15.00/call with discounts if used more than once per week. Courtesy Visits are calculated at $30.00/visit with discounts if used more than 5 days per week.
In the end you are looking for options to get home after getting sick or help your loved one get home and rejuvenate. Synergy HomeCare is in a position to provide you with options that may fit your limited budget and help you get well soon!
