Care Manager

Overview

SYNERGY HomeCare of North Seattle is seeking a proven Care Manager who is responsible for ensuring that referral sources and clients are happy, satisfied and are receiving the highest level of support services and continuity of care. This position can be full or part time and paid a salary or hourly wage.
